Co-Ex Grant Completion Form

Before beginning the Grant Completion form, please gather:

1. The number of students who participated in the service project and number of hours of service completed

2. A copy of the receipt(s) for the items purchased

3. Pictures of the students’ service project and of them using the items purchased

4. Testimonial from staff and students about the impact of the grant funds and/or the service project.

 Please note that service projects must be completed and Grant Completion Forms submitted by the end of the academic year in which the items are received, or the grantee will not be eligible for a grant the following year.
